Office Policy

All patients will be expected to pay any co-pay and deductibles when services are rendered. Patients who have no insurance coverage will be expected to pay in full at time of service.

Northwest Arkansas Gastroenterology Clinic does except payment for services rendered at time of your appointment.

1. Patients with Insurance coverage including Medicare and Arkansas Medicaid will present their ID card at the initial visit. You will be expected to pay any deductible and co-pay. Medicaid Recipients will need to have referral from PCP.

2. Patients who have an Insurance package that is an HMO or require a PPO will need to have the written referral from their Primary Care Physician, if required by your Insurance Company. Deductibles and Co-pays are due when services are rendered.

3. Self-pay patients will be expected to pay for their visit in full at the scheduled appointment unless prior arrangements are made.
